Logo: Scrolling up in the credits of Dark Shadows, we see a superimposed logo with stylized text (the first letters of each word are in boxes) as follows:

FX/SFX: Scrolls up in the credits.
Music/Sounds: The end theme of Dark Shadows, that is a spooky dark theme, and/or a deep voice-over with a echo/stutter-like effect saying, "Dark Shadows is a Dan Curtis Production."
Availability: Found on all episodes of the original Dark Shadows TV series, which ran on ABC from 1966 to 1971.
Editors Note: The theme/announcer is notable for being ominous.
2nd Logo
(February 21, 1973)
Logo: On an extreme close-up of a scary vampire face, there is an in-credit text that reads:
The logo would then cut to a Metromedia Producers Corporation logo that would be superimposed on the same background.
FX/SFX: None.
Music/Sounds: The end-title theme, which is that of The Norliss Tapes, a very dramatic music composition.
Availability: Seen on the 1973 TV movie The Norliss Tapes.
Editors Note: This logo is somewhat notorious for being scary to many people thanks to the scary music and look of the eyes.
3rd Logo
(January 13-March 22, 1991)
Logo: An in-credit text that reads as "DAN CURTIS PRODUCTIONS, INC." with the text "IN ASSOCIATION WITH" in a smaller font below.
FX/SFX: None.
Music/Sounds: The end-title theme.
Availability: Rare. It was only seen on the short-lived 1991 revival Dark Shadows.
